Job Description:
A company is looking for a Licensed Practical Nurse (LPN) with
Illinois licensure for a remote clinical position.
Key Responsibilities:
Manage patient census and address gaps in clinical and non-clinical
care
Coordinate community resources and provide patient education for
chronic conditions
Assist in medication management and ensure timely delivery of
necessary services
Required Qualifications:
Active Illinois LPN License
At least 3 years of direct patient-facing work experience
Technical proficiency in Microsoft Office Suite
Experience with chronic care management
Ability to work in a HIPAA-compliant remote environment
